An initial visit with a provider involves a medical history review, possible physical examination, and possibly diagnostic test ordering to evaluate a patient's health, identify issues, and develop a treatment plan.  Several questionnaires will be sent and also gone over individually at appointment.

A medication refill appointment with a provider typically involves a brief review of his/her existing patient's medical history, confirming the need for the medication refill, and issuing a new prescription. It may also involve evaluating any side effects or concerns related to the medication and ensuring that the treatment plan is still appropriate for the patient's needs.

I see ADHD, depression, anxiety, bipolar, and many other disorders.  I enjoy seeing the improvement in patients and helping people to be the best they can be.  I work hard to fully evaluate individual and at every session time is spent following up thoroughly to be sure treatment is going well.  Communication is essential so I use a combination of pre-appointment forms along with video for evaluations.

Family and Psychiatric Mental Health Nurse Practitioner

An initial visit will involve obtaining a medical history and filling out, online, forms for different disorders.  These forms will be gone over at the visit along with more questions asked to obtain full information for making a diagnosis and creating a plan of care.  Education is an important part of the care I provide this every session. Every patient is unique in their symptoms, and I believe in treating everyone as a unique individual working together to devise the best plan of care.

Follow-up visit 

For existing clients who may be in need of medical care that is new such as rash, pink eye, cough, ect.  Also for existing clients who may need a change and want to discuss in a longer appointment.

EXISTING Client Medical Consultation or Psychatric concern

It may also involve evaluating any side effects or concerns related to the medication and ensuring that the treatment plan is still appropriate for the patient's needs.  This is a form only service for existing stabilized patients. For each video visit there will now be a change to one refill between visits.
