Hello, I'm Chioma Momife, a board-certified psychiatric nurse practitioner with over 13 years of experience in mental health care. I specialize in diagnosing and treating children, adolescents, and adults, providing a range of services from medication management to holistic alternatives like lifestyle modification, nutrition, and stress management. I believe in a mind-body connection and use this approach to create personalized treatment plans for my patients. • Apply evidenced-based pharmacological and non-pharmacological therapy in the management of psychiatric disorders under the supervision of the psychiatrist
• Reviewed the client's medical history, ordered laboratory tests, interpret the results of diagnostic tests, and observed client behavior for accurate diagnosis of illness
• Provide follow up visits for clients and monitor any beneficial and side effects of medications and adjusting where necessary
• Collaborate with members of the interdisciplinary team (physicians, PMHNPs, PAs, social workers, psychologist, etc.) to coordinate care
• Educate the client about their disease condition and the treatment plan
• Demonstrate good judgment by providing referrals to higher level of care e.g., psychiatric emergency centers, and psychiatry inpatient services
