Unique Minds Psychiatry is a telehealth private practice dedicated to serving individuals in need of mental health services. I am the owner and practice provider at UMP, and I am a dedicated patient-focused private practice provider that is actively involved in bridging the gap in underserved communities, while improving the recovery rate for clients with mild, moderate to severe mental health challenges such as depression, bipolar disorder, schizoaffective disorder, anxiety disorders, adjustment disorder, and ADHD. 

Founder/CEO-PMHNP-BC

Psychiatry and Medication Management services via telehealth. Consist of work with patients suffering from chronic and acute psychiatric impairments, intellectual impairments, and cognitive impairments in a telehealth setting. Promoting mental health awareness, helping individuals better understand their mental health issues. Providing evaluation, diagnosis, and medication management of the full spectrum of acute/chronic mental health disorders including ADHD. Formulating individuals plans of treatment, which involves prescribing medications, providing supportive therapy, detailed patient evaluations, lifestyle adjustments, and behavioral interventions. Completing a comprehensive review of the patient’s medical history to assist with making informed decisions and guide the creation of an individualized plan of treatment that addresses each patients unique needs.

PMHNP-BC

92 bed Psychiatric hospital. Work exclusively with patients suffering from acute psychiatric impairments, intellectual, and cognitive impairments. Provided evaluation, diagnosis, and medication management for acute/chronic mental health disorders for up to 25 adult patients. Conduct evaluations and collaborate with the psychiatrist and other disciplines to develop and adhere to treatment plans as part of a collaborative multidisciplinary psychiatric team. Provided on-going healthcare and educational sessions to staff and family about caring for patients with various psychiatric disorders. Common patient profile included depression & suicidal ideation, bipolar disorder, schizophrenia, anxiety, substance abuse, and personality disorders. Always prioritized goals of stabilization and prevention of readmission.


Traveled to various long-term care facilities. Psychiatric care and services for 12-15 residents. Assessment of mood, behavior and thought processes, crisis intervention and de-escalation, safety assessments, managing individualized treatment plans and interventions, diagnostic testing, tracking of medication administration. Collaborate with other disciplines such as medical providers, nurses, therapists, and other members of the collaborative multidisciplinary team.


Level I Trauma Center with 1.1k beds. Currently working with the central staffing department PRN on various critical units. Initially began on the medical ICU on a fulltime basis and transferred to the cardiac care unit after just 1 year to treat higher acuity patients. Gained experience in cardiac interventions and supplied advanced respiratory support. Acute interventions with CCU patients included continuous dialysis, ballon pump operation, ABG interpretation, hyperthermia protocol, and stabilization of hemodynamics. Brought focus on psychiatric health to bridge the gap in patients with dual medical/psychiatric diagnosis.

ICU-RN

Level III Trauma Center and 595-bed hospital. Transferred to a more acute higher level of care facility to gain more knowledge and experience. Known for leadership skills and an understanding of management, professional nursing practices, and managed care as a primary driver of high- quality patient care. Key objectives ranged from discharge planning to minimizing length of stay and upholding high standards of care. Collaborated with social work, patient families, and providers. Awarded Daisy Award for exceptional patient care.
HCA-East Houston Medical

RN-ICU

120- bed community-based hospital. Started on the Dialysis Unit and moved into the ICU after communicating previous ICU experience and demonstrating deep practical knowledge and understanding of treatment modalities in both critical care and nephrology. Monitored multiple IV drips and medication administration on lower acuity critical patients. Demonstrated skills in patient assessments, adhering to treatment protocols, teamwork, and thorough documentation in EHR systems.


RN, ICU/ Acute Dialysis RN

556- bed hospital. Pursued advanced training and moved from working exclusively with acute dialysis patients to critical care patients in the ICU. Provided continuous hemodynamic monitoring of critically ill patients intubated and on ventilators for respiratory support as well as interpreted heart rhythms.
Served as an educational resource to patient families and assessed patients for sudden and/ or subtle changes in medical condition.

Outpatient dialysis center. Member of an acute care team traveling between hospitals providing contract dialysis treatments. Dialyzed up to 11 patients a day in an extremely high-volume, direct treatment administration role. Responsibilities included providing hemodialysis treatments to patients with chronic/acute renal failure by accessing sites such as hemodialysis catheters and cannulation of fistulas and grafts. Adjusted dialytic therapy based on patient treatment response and closely evaluated variances/adverse reactions to care.

Acute Dialysis RN

590- bed hospital. Began in telemetry in an acute care role. Admitted pre and post- surgical cardiovascular patients, conducted assessments, administered medications, completed lab draws and interpretation, and collaborated in a multidisciplinary environment with various specialties including respiratory, wound care, physical therapy, and cardiology.


RN, telemetry

65 bed Psychiatric Hospital. Duties included care for 12-15 patients, assessments of mood, behavior and thought processes, crisis intervention, de-escalation, application and monitoring of 4-point and body net restraints for safety, managing treatment plans and interventions, communicating test results with physicians, medication administration, documentation, and post-treatment home care education.


RN, Acute Psychiatric Care

Long-term care facility. Brought on as an LPN and promoted to Charge Nurse. Supervised 1 LPN and 5 CNAs and managed a 60-bed unit. Closely monitored patient care and medication administration, and successfully prevented adverse reactions. Mentored CNAs in proper care, documentation, and charting as well as upheld a high standard of patient care.

An initial visit with a provider involves a medical history review, an assessment of your specific symptoms and concerns, and possible diagnostic tests to evaluate a patient's health, identify issues, and develop and discuss treatment options.

Psychiatric Evaluation

A follow-up visit with a provider involves checking the progress of the patient's treatment plan, assessing any new symptoms or concerns, and adjusting the treatment plan as needed to ensure optimal health outcomes.

An opportunity for potential patients to discuss their concerns or needs with a provider, or therapist, to determine if their services are a good fit and to receive initial advice or guidance. Many providers offer free or very affordable consultation services with potential patient leads. Providers who add Free consultation as a service receives 2-3 times more prospects.
