Adult/Gerontology Primary Care Nurse Practitioner

    • Chronic and acute care management in correctional facility.
    • Chronic care management such as type 1 and 2 diabetes, hypertension, hyperlipidemia, CHF, A-fib, etc.
    • Acute illness management to include wound debridement, minor burns, otitis, fluid replacement, toenail removal, skin biopsies and more.
    • Administered COVID-19 vaccines to over 400 inmates.
    • Joint injections.

    • Provides comprehensive medical care to patients living in ALF, SNF and LTC of which many are frail.
    • Timely and frequent assessments of changes in health status to optimize and maintain well-being and preserve quality of life.
    • Provides age-appropriate preventative care including flu shots and immunizations.
    • Regularly and carefully reviews all medications, including prescription, OTA, herbals and supplements to minimize polypharmacy and prevent 
       adverse medication interactions.
    • Conduct comprehensive assessment in person and via Telehealth.


An initial visit with a provider involves a medical history review, an assessment of your specific symptoms and concerns, and possible diagnostic tests to evaluate a patient's health, identify issues, and develop and discuss treatment options.

A follow-up visit with a provider involves checking the progress of the patient's treatment plan, assessing any new symptoms or concerns, and adjusting the treatment plan as needed to ensure optimal health outcomes.

Follow-up Visit

A prescription service appointment with a provider typically involves a brief review of his/her existing patient's medical history, confirming the need for the prescription service, and issuing a new prescription. It may also involve evaluating any side effects or concerns related to the medication and ensuring that the treatment plan is still appropriate for the patient's needs.

Prescription service

An opportunity for potential patients to discuss their concerns or needs with a provider, or therapist, to determine if their services are a good fit and to receive initial advice or guidance. Many providers offer free or very affordable consultation services with potential patient leads. Providers who add Free consultation as a service receives 2-3 times more prospects.
