An initial visit with a provider involves a medical history review, an assessment of your specific symptoms and concerns. Initial evaluation/assessment mental health disorder including, but not limited to bipolar disorder, anxiety, depression, substance use disorder, PTSD, amongst others. ****This does not include ADHD assessment.****

Diagnosis, management, and treatment of mental health disorders through medication-focused treatments, delivering personalized medication strategies to enhance patients' mental well-being.

A follow-up visit with a provider involves checking the progress of the patient's treatment plan for anxiety, depression, bipolar disorder, amongst others. This visit does not include ANY controlled medication prescribing. 

An initial visit with a provider involves a medical history review, an assessment of your specific symptoms and concerns. If you were previously diagnosed with ADHD and have documentation, DO NOT schedule this appointment. Schedule ADHD Management appointment and please send documentation of diagnostic assessment prior to the start of your visit. If you do not have supporting documentation, schedule this appointment for reassessment.

A follow-up visit with a provider involves checking the progress of the patient's treatment plan, assessing any new symptoms. Schedule this appointment for follow up and refill of non-controlled medication in the treatment of ADHD or other mental health conditions. 
****THIS VISIT DOES NOT INCLUDE PRESCRIBING FOR ANY CONTROLLED MEDICATIONS****

An initial visit with a provider involves a medical history review, an assessment of your specific symptoms and concerns.

Focuses on the diagnosis, treatment, and management of infectious diseases; managing acute and chronic infections, providing vaccinations, and offering travel health advice.

A follow-up visit with a provider involves checking the progress of the patient's treatment plan, assessing any new symptoms or concerns.

A monthly follow-up visit with a provider involves checking the progress of the patient's treatment plan, assessing any new symptoms or concerns for substance use disorder or for management and prescribing of Buprenorphine.  This does not include controlled medication prescribing for any other medications.

Issues secondary to pharmacy issues and needing medications to be resent to a different pharmacy. Every visit for new or established patients has a one-time complimentary prescription change to a different pharmacy. If extensive follow up is needed or medications need to be sent more than once to a different pharmacy, an appointment is required.

A 30-minute follow-up visit with a provider involving reviewing therapy skills in managing mental health symptoms. This visit does not include medication management. 

A visit to review and complete requested paperwork. This includes letter of accommodations for work or school, FMLA paperwork for work, or Emotional Support Animal paperwork.
