I provided care to adult with acute illness, chronic conditions, and mental health conditions primarily telemedicine via Klarity, Kiwi, Mishe, Sesame platforms.

I provided care to adult and pediatric patients with acute illness, chronic conditions, and mental health conditions. I also do physicals, std screenings, Covid testing, and Covid screening. In addition, I triage sick and high-risk patients using call visits or telehealth visit, in order to prevent patients from being admitted to emergency department. I have started DSME program (Diabetes Self-Management Education) which managed, coordinated, and educated diabetic patient and improved their outcomes.

A follow-up visit with a provider involves checking the progress of the patient's treatment plan, assessing any new symptoms or concerns, and adjusting the treatment plan as needed to ensure optimal health outcomes. (2 or less complaints) ( Example: follow up on diabetes, cholesterol, thyroid disease, high blood pressure;  gout, flu like symptoms, covid symptoms, asthma, sinus infection, sore throat, pink eye, acid reflux, constipation, back pain, knee pain, ankle pain).

Follow-up visit - Urgent Care/ Sick Visits

A medication refill appointment with a provider typically involves a brief review of his/her existing patient's medical history, confirming the need for the medication refill, and issuing a new prescription. It may also involve evaluating any side effects or concerns related to the medication and ensuring that the treatment plan is still appropriate for the patient's needs. Medication refills on stable/controlled symptoms/conditions/ Forms/ Referrals to specialists - $50

A follow-up visit with a provider involves checking the progress of the patient's treatment plan, assessing any new symptoms or concerns, and adjusting the treatment plan as needed to ensure optimal health outcomes.
