Compassionate and understanding therapist

Provides short-term crisis intervention and case management to adults experiencing dual-diagnosis and multiple psychosocial stressors. Completes brief psychiatric assessments to determine medical necessity for admission into psychiatric triage. Collaborates and consults with professionals of diverse educational backgrounds including psychiatry, MFT’s, Psychology, and Social Workers to provide best care to client population. Provides assistance to psychiatrists and medical staff with discharge planning of clients and works with client’s families with arranging aftercare services. Helps secure placement for clients into A&D treatment facilities and homeless shelters. 

Social Worker II

I interned in Primary Care Mental Health Integration within the Loma Linda VA which integrates care for veterans’ physical and mental health conditions, improves access and quality of care across spectrum of illness severity. My responsibilities included providing individual therapy utilizing mainly CBT for patients experiencing mental health conditions and psychosocial stressors, facilitated psychoeducation group for stress & depression for three rotations.  Worked alongside with an interdisciplinary team in a Diabetes group medical appointment providing brief therapy for veterans experiencing psychosocial stressors.  I observed clinical social workers complete short assessments and screening for PTSD in the triage clinic.  I participated in a interdisciplinary group supervision of psychologists clinical social workers. Consulted with various providers and CPS to assist me with my caseload. Master of Social Work Intern

Social Action Community Health System: 09/2011- 06/2013
SACH Norton provides primary health care, dental care, pediatrics, behavioral health, and case management for mothers with children less than 6 years of age and mainly serves low-income and Hispanic clients. I interned for two rotations in the family resource center, and I interned in primary care with Dr. Hart. Provided individual therapy for one patient throughout the academic year and met with individual patients providing case management through the family resource center. Provide case management services for families and individuals. Responsible to work towards family stabilization, provide ongoing assessment and support to clients. Assist clients to utilize community resources to meet individual needs; Co-facilitated Parenting Wisely. Helped clients with linking them to resources within the community. Make appropriate referrals to appropriate agencies. Collaborate with internal physicians and health care providers to access 

Developed and facilitated goal-oriented psychoeducation and therapy groups to improve residents' quality of life. Focuses primarily on psychoeducational and coping skills during group therapy. Provides suicide and crisis risk assessments and interventions as needed. Participate in risk assessment, evaluation, and for alternate level of care placement within the detention center. Completes general mental health assessments Conducts assessments and formulates diagnosis of individuals if applicable. Develops, monitors, and modifies treatment plans in collaboration with clients expressed goals. Presents clinical documentation and observations to interdisciplinary team. Licensed Clinical Social Worker

Provided individual therapy primarily to children, and adolescents ages 5 and up.

Works with the Behavioral Health Interdisciplinary Team (BHIP) to provide individual therapy, crisis assessments, to veterans of different service backgrounds, gender, and different deployment tours.  

Senior Social Worker

Works in a team dynamic with another clinician or support staff member i.e Mental Health Specialist, Social Worker II, or Peer Family Specialist as a first responder in a 24 hour mobile crisis response team. Completes psychosocial assessments, suicidality/homicidality/ and grave disability risk assessments on children and adults. Works with various agencies in the West Valley Region of San Bernardino County i.e Police and Sheriff's Departments, Elementary, Middle, and High Schools, local hospital ER's, Urgent Cares, Mental Health Outpatient clinics, and members of the community to complete psychosocial, suicide, homicidal, grave disability risk assessments. Writes 5150/5585 psychiatric holds for individuals facing mental illness who are either in danger of hurting themselves, others or who are gravely disabled as a result of a mental illness. Assists with transporting consumers to psychiatric hospitals on voluntary/involuntary holds. Links consumers to appropriate referrals.

Clinical Therapist I
