Responsible for Utilization Review Management daily. Participate in interdisciplinary rounds and meetings. Discharge planning for patients. Collaborate with liaisons from the community, HHC, Social workers, case managers, Skilled nursing facilities. Mentor UCLA Nursing Students. RN Case Manager

Conduct Physical Health Evaluations. Teaching patients about TRT treatments, Vitamins, Peptides or medications administration, side effects, and indications. Responsible for documentation of physical health evaluations and make recommendations based on findings if appropriate for treatment.

A medication refill appointment with a provider typically involves a brief review of his/her existing patient's medical history, confirming the need for the medication refill, and issuing a new prescription. It may also involve evaluating any side effects or concerns related to the medication and ensuring that the treatment plan is still appropriate for the patient's needs. (non-narcotic only, some medications may be limited with telehealth)
