•	Provided Telehealth psychiatric evaluations and medication management to patients in skilled nursing facilities along with adolescent and adult psychiatric hospital units.
•	Ensured continued quality follow up care by monitoring, recording and evaluating psychiatric conditions.

•	This role consisted of providing psychiatric consultation to those with known or suspected mental health concerns in their homes through referrals from Landmark's interdisciplinary team members.
•	The population I work with is an identified cohort of those with at least 6 or more complex health conditions in the geriatric population.
•	This position collaborates with both Landmark and community health care providers to assist patients in overcoming barriers to treatment.
•	Connecting patients to community behavioral health providers and providing complete diagnosis and treatment services, evaluating their current treatment and offering recommendations to optimize outcomes, or monitoring for changes from baseline and/or adverse effects from treatment.



•	Developed and implemented new strategies and policies to establish long-term business objectives and provide strong and sustainable organizational leadership.
•	Hired and managed employees to maximize productivity while training staff on best practices and protocols.
•	This includes but is not limited to responsibilities such as developing the clinical policies, assisting with licensing and credentialing other Nurse Practitioners to provide telepsychiatry consultations to skilled nursing facilities in New Mexico, Iowa, and Maine.
•	Throughout this process I was able to acquire facility contracts based on referrals from companies I had already been providing services to.
•	Cultivated strong relationships with external partners to foster collaboration and maximize resources.

Owner/Practitioner

My role as a Psychiatric Mental Health Nurse Practitioner at this company provided vast experience with adults ages 18 and older due to the various settings I worked in simultaneously. During this time I saw patients at nursing homes, and in the outpatient office setting for medication management and evaluated the effectiveness of Transcranial Magnetic Stimulation (TMS) treatment.
I also had hospital admitting privileges at Sacred Heart Hospital (SHH), where I provided patient care from the initial diagnostic evaluation, and adjusted medications as clinically indicated, through to discharge. The population served at SHH included acute care for younger adults, acute care for older adults, and extended acute care for younger adults.
