Family Nurse Practitioner
Reyes Family Health was founded in 2021, as a result of the first wave of the coronavirus pandemic. The company was founded by Joana Reyes, a single mother of two and a family Nurse Practitioner with over a decade of experience in the nursing field. The mission of the company is to provide accessible and affordable access to care to the community, primarily using telemedicine. Ms. Reyes is family oriented, she enjoys dedicating her time to compassionate care in the nursing field and also loves spending quality time with her two children. The company's logo, designed by her sister, consists of two cherry blossoms which symbolizes her two children, with hopes that they will continue the life values and philosophy she instilled in them. The butterfly symbolizes her father, who passed away suddenly and tragically at thirty-two years old, when Ms. Reyes was just five years old. Her father is the primary inspiration that led Ms. Reyes to dedicate her life to serving the community in the nursing field. As much as she enjoys working as a Nurse Practitioner and being a dedicated mother, Ms. Joana Reyes is truly grateful for her family, friends and patients for their support and trust with their care.

Joana Reyes is a Board-Certified Nurse Practitioner. She earned her Bachelor of Science in Nursing with Magna Cum Laude from Molloy College in 2010 and her Master of Science in Nursing from New York University in 2015. As a Nurse Practitioner, Joana has over 8 years of experience in a large outpatient practice in New York treating patients with acute, chronic, and mental health problems. She has also worked with several telehealth and started a successful diabetes program, using meal planning and medication management. Patient care is Joana’s passion and calling. She strongly values providing care with compassion, empathy and understanding. Because of her qualities, she has developed enduring bonds with patients, showing strength through her patient care relationships.

Follow-up visit - Urgent Care/ Sick Visits
A follow-up visit with a provider involves checking the progress of the patient's treatment plan, assessing any new symptoms or concerns, and adjusting the treatment plan as needed to ensure optimal health outcomes. (2 or less complaints) ( Example: follow up on diabetes, cholesterol, thyroid disease, high blood pressure; gout, flu like symptoms, covid symptoms, asthma, sinus infection, sore throat, pink eye, acid reflux, constipation, back pain, knee pain, ankle pain).
